Payment, Billing, Cancellations

  • Illuminate Psychiatric Services does not accept any type of insurance.

  • Payment in full is expected at the time of service.  Once payment is received a receipt can be provided.

  • A new patient appointment may be cancelled without charge up to 24 hours before the scheduled appointment.  Same day cancellations will be charged 50% of the new patient appointment fee.  If you do not call to cancel or do not show up for your scheduled appointment, you will be charged the full evaluation fee.

  • If for some reason payment is not made at the time of visit, payment will be required before further appointments can be scheduled.

  • Illuminate Psychiatric Services requires at least 12 hours notice to cancel/change an appointment.  If adequate notice is not provided, 50% of the appointment fee will be charged.  The fee will be due at the next appointment.

  • Accounts that are seriously overdue may be forwarded for collection activity.

  • Illuminate Psychiatric Services accepts cash, check, debit, and major credit cards.  A processing fee will be applied for any/all debit/credit card transactions at the patients expense.  A $30 NSF fee will be added to all returned checks.

Patient Refusals and Dismissals

  • Illuminate Psychiatric Services reserves the right to decline acceptance of any new patient without repercussion.

  • Illuminate Psychiatric Services reserves to right to dismiss any patient from our practice for reasons including, but not limited to:

    • Disruptive behavior, missed appointments, drug-seeking behavior, or incompatible provider-patient relationship

Prescription Medications

  • Illuminate Psychiatric Services will prescribe enough medication to last until your next recommended visit.

  • It is the responsibility of the patient to track their medication supply, remaining refills, and request refills during appointments when needed.

  • If an appointment is changed or canceled, the patient is expected to reschedule the appointment to ensure an adequate supply of medication.  In the event an appointment cannot be made before a prescription runs out, it is the patient responsibility to request a short-term refill.

  • Prescriptions for controlled substances will ONLY be provided during appointments.

  • If a patient is prescribed a controlled substance he/she will need to be seen at least every 3 months. No exceptions.

  • Illuminate Psychiatric Services subscribes to the Indiana Prescription Drug Monitoring Program and adheres to the State of Indiana Controlled Substance Policy.

Release of Medical Records

  • Illuminate Psychiatric Services respects your privacy.  Your information will not be disclosed to others unless:

    • You sign a release instructing disclosure

    • The law authorizes or requires it

**By scheduling an appointment with Illuminate Psychiatric Services you are agreeing to all of the above policies.

Appointments and Communication

  • Appointment cancellations/changes require at least 12 hours notice to avoid payment of 50% of the appointment fee.

  • Late Arrival: Arrival to an appointment more than 10 minutes past the appointment time will be considered a missed appointment or a no show and charged accordingly.

  • Illuminate Psychiatric Services is not available 24/7.  We will do our best to respond to patient phone calls, emails, and secure messages during normal business hours.

  • In an emergency patients should call 911 or go to the closest emergency room.

Privacy and Confidentiality

  • Illuminate Psychiatric Services fully abides by the federal Health Insurance Portability and Accountability Act (HIPAA) and Indiana state law related to health care access and disclosure.

  • Protected Health Information (PHI) includes any information in the medical record that can be used to identify an individual including diagnoses, results, treatment, etc., and payment relating to these services.
